Housekeeping git meta and tests, custom issue templates #972

Add custom issue templates, to make everybody's lives easier. (Mindfully borrowed and extended from the https://github.com/doctrine/dbal project)
Clean up readme a bit and make it prettier.
Add .gitattributes file to ignore certain files on export.
Simplify the code checking and testing by using defaults/standards for filenames.
