refactor(npm.js): pass remaining npm.js references

There are still a few files in lib/utils that require npm.js directly. These were part of npm.js and cli.js themselves, and did not need to be updated as part of the larger npm context refactor.  This is the second half of the "pass npm.js context" refactor.
