[cppvsdbg] Breakpoints are set in all files with same name

Support setting breakpoints in a single file. This requires vsdbg to validate the file it is setting a breakpoint in with a checksum to prevent setting breakpoints in the wrong file.

A quick workaround is to add a condition to your breakpoint that will only be true/defined in the file you're interested in. Just the name of a local variable can work.

crazy this 'feature request' is still open from 2017... glad i stopped using vscode for C++.
