In the context of creating the Kubeflow Community Distribution the Slack channel was renamed from `#kubeflow-platform` to `#kubeflow-community-distribution`. While fixing this reference in various places (excluding the historical release files) also make some drive-by fixes regarding the Markdown formatting.
