Fix Infinite Change Event in Browser History

Closes: #12579

Not quite sure why this worked without additional_inputs but this is the fix.

Moreover, there was a bug in how we calculate the conversation title. We're loop through the messages and break when we find the first user message. Since the conversation history is now a list of lists, this break statement was not working as expected.
