Error in the chat after adding / using Sonnet 4.5

Bug Description:

When using Sonnet 4.5 with Theia Coder, you get occasional errors (shown in the chat):

400 {"type":"error","error":{"type":"invalid_request_error","message":"messages.18: tool_use ids were found without tool_result blocks immediately after: toolu_01FKGMo3E7BZNryEuGZGQGhg. Each tool_use block must have a corresponding tool_result block in the next message."},"request_id":"req_011CTgAiXKxVHHzKqwX4fa1U"}

Steps to Reproduce:

1. Add Sonnet 4.5 to the settings and configure it to be used in the Agent Configuration
2. Task Coder to do something
3. Observe the error above (not always)
