Improve tracking n caching logic

Fixes and improves the caching logic and the tracking logic. In particular fixes:

1. There was some issues with managing subscriptions of the same user for different tokens
2. There was caching issues when only part of the tokens being queried are cached
3. Make sure we stop keeping track of some tokens when clients disconnect

Test

You can create multiple windows with different token selections (like in the screenshot) and make sure you get the relevant updates
