Remove calypso protocol

The calypso protocol is not needed, as the calypso service is not available. Only the calypso contracts should be available.

If somebody needs calypso, she'll have run a set of calypso nodes herself.

Eventually add a compile or runtime option to enable/disable calypso services.

Same goes for dkg/pedersen, which is only needed in calypso.
